When you open this four panel folder the page on the left is for recording the following information: This keepsake was given with love... to. The panel on the right is for recording the dates this folder was started form the date of birth through the baby's fifth birthday.

When the two panels are again opened into the four panels there is an information page on both the left and right side of the folder. The information page on the left requires the following information: Songs we listen to. Movies we enjoy. Hit TV shows. Sports heroes. The front of the information page on the right side of the folder requires the following information: My Name. Birthdate. Time of Birth. Height. Weight. Color of Eyes. Color of Hair. I looked like... because. How my name was chosen. Other names Mom and Dad considered. The reverse side of this information page provides an area to fill in the family tree.

The four panels inside the folder have ports for six different year sets of coins; the birth year, age one, age two, age three, age four and age five. This four panel folder measures 9 1/2" high x 7 1/8" wide when closed.
